Once
you have registered and paid on-line please bring the verification
documents to the Milpitas Sports Complex baseball field on 1/19, 1/20,
1/26 or 1/27 between 10:00am-2:00pm.

1. Little League Medical release-completed and signed
2. Parent/Volunteer Code of conduct-completed and signed
3. Birth Certificate for verification only
4. Little League School enrollment form (recommended if your child attends a school in Milpitas) OR 3 proofs of residency- requirements can be found at found at https://www.littleleague.org/university/articles/residency-requirements.
